Access to Records

In accordance with Louisiana Department of Health, Health Standards Section. Loyalty Behavioral Counseling notifies the public availability of records for inspection and copying.

Records Available: Medical records, assessments, and treatment plans.

Hours of Access: Monday-Thursday 10 am-3 pm

Request Process: Individuals wishing to access records may submit request by email and allow 3-5 days to receive records.
