Notice to the Public: Effective at 11:59 p.m. on September 16, 2024, Oil Center Surgical Plaza located at 1000 W Pinhook Rd, Lafayette, LA 70503 will surrender its Ambulatory Surgery Center license. Ochsner Lafayette General Surgical Hospital (OLGSH) will then open an extension of the hospital campus at the 1000 W Pinhook Rd, Lafayette, LA 70503 facility in Suite 100 and Suite 204 effective as of 12:00 a.m. on September 17, 2024, with the goal that there be no disruption in patient services from the surrender of the Oil Center Surgical Plaza licensure. Individuals with questions regarding access to medical records for services performed at Oil Center Surgical Plaza may contact OLGSHs medical records department at 337-289-7771 option 2 or 337-289-7970 for radiology films. Release of Information is located at 900 E. St. Mary Blvd 1st floor.
